  • Please complete the front and last page (your personal details) but do not sign it.
  • Print you full name on the bottom of every page and add you Date of Birth in every box.
  • If you wear glasses or contact lenses for driving, please bring them with you and you must be able to remove (and replace) your contact lenses; if applicable
  • Bring a list of medication (if applicable)
  • If you have had hospital treatment or you are under the care of a specialist, please bring details with you (dates of attendance, hospital letters etc)
  • Bring some Photo ID
  • You will not need a urine sample.

Medicals Without the Fuss

We have been providing HGV, PSV, Taxi and Motor Sport medicals for over 15 years.

We conduct a comprehensive examination and where required a full eye test up to Group 2 requirements.
